I, the undersigned parent or legal guardian of the child named above, acknowledge and agree to the following:

I understand that this class may include gentle movement activities, such as children’s yoga, stretching, and mindfulness exercises. While these activities are designed to be safe and age-appropriate, I acknowledge that participation involves a degree of physical activity and carries a small risk of injury.

I agree to inform the instructor of any medical conditions, allergies, injuries, or physical or developmental concerns that may affect my child’s ability to participate safely.

In consideration of my child’s participation in the program, I voluntarily waive, release, and discharge Lisa (Xia) Li, Tangerine Kids & Tangerine Yogis, and any associated instructors, contractors or staff from any liability, claims, demands, or causes of action arising from any injury or damages related to my child’s participation, except where such liability cannot be excluded under law.

I understand that all reasonable care and supervision will be provided to ensure a safe and supportive environment for all children.
